Just imagine this game if everyone had a second city.... Not good!
You don't have to pay for the 2nd city if you don't want to. You can win one on Merlin's Chance. I've won two that way so far. You can attack pict camps, level 6 and above, and get enough crests to create a 2nd city deed. Oh another option is to get enough free gems via Tapjoy and buy it that way. Lots of choices.
